This resource has been created by the 340B Prime Vendor Program to assist stakeholders by combining the most recently available 340B-specific Medicaid data with publicly available federal data. Please note that additional requirements or contractual obligations that may be in place from third-party payers are not included in this resource.

Contract Pharmacies
State Permits 340B via Contract Pharmacy
Beginning 9/1/2026, 340B claims submitted by a provider on behalf of a 340B grantee (as defined in ORC 5167.01), must be submitted with a Submission Clarification Code = 11 (NCPDP field 420-DK), in addition to being submitted with a Submission Clarification Code = 20 as required above.340B claims submitted by non-340B covered entities (“contract pharmacies”) not being submitted on behalf of a 340B grantee will be denied with instruction to usenon-340B drug stock in accordance with ORC 5167.123.340B claims submitted directly by a grantee (through the grantee’s in-house pharmacy) are not required to contain a Submission Clarification Code value = 11
